Make a request for the medical records in writing on the form the facility where it was given uses. It is usually called a HIPAA release for records. But you should now that in many instances, the manufacturer is noted recorded in a patient's medical chart, nor do most medical providers list the lot and batch number. You didn't specify when your daughter had her reaction. If it was between November 2007 and today, you should report the incident as a consumer to the FDA - www.fda.gov.

do you know how I could find out what type of heparin my husband received when he was in the hospital feb.2006. He had cervical surgury c3 to c7, had trouble getiing off the vent, so he was given heparin to prevent bloodclots, since he couldn't get up & move around. he was on the vent in induced coma from feb. 3rd. until feb.15th. we were told his blood thinned too much and caused a hematoma on the spinal cord resulting in spinal cord damage. I just was reading an article in our local paper about the heparin problems. the night before it was discovered his lood pressure also dropped to a critical low.
